AS far as I know, no insurance will cover this unless you can show medical necessity such as pulmonary problems, back problems, etc. Any insurance company you contact will tell you what procedures their policy will cover. Your doctors office will help you.

The insurance I have has coverage for professional services (the doctor) and the facility (the hospital) which would cover breast reduction surgery. Whether or not they will cover the surgery for ME is dependant upon if I meet the criteria. You will want to ask about any waiting periods or exclusionary periods, and then ask about the benefits "is the surgery subject to my deductible?" (which means I pay for it until my deductible is met). Hope that helps.
