I had left hemilaminectomy almost 10 years ago at L5-S1. For the past 1-2 years occasionally I would have a burning sensation above the surgical site but did not seek medical intervention. For the past 2 months I have had pain in my left hip starting at the pelvic area with weight bearing pain as well. My MRI was as follows: Findins: Normal alignment. There is no marrow edema or compression fracture. The disc heights are intact. there is diffuse disc desiccation.l The conus and the cauda equina appear normal.
At L2-L3 there is a small central disc extrusion extending posterior to the L2 vertebral body. there is mild theca sac indentation. L3-L4 there is central annular tear and mild diffuse annular bulge.
At L4-L5 there is tiny annular tear without disc herniation.
At L5-S1 on the left side, a small disc herniation is suspected. There is perineural scarring within the left lateral recess related to prior right hemilaminectomy. Post surgical changes noted within the left lateral recess.
If clinically warrented consider CT myelogram.
I run agility with my dogs. Is this safe to continue to do right now?
Can you please explain what this all means and what the treatment plan would be. Is physical therapy and or chiropractic treatment of any value? I am not looking forward to a 2nd surgery.
What is the most minimally invasive choie if surgery is indicated.
