I fell off a bike onto the sidewalk and the next day I noticed some inflamation around my middle finger nail cuticle (left side). I tried over the counter medication (polymyxin B sulfate/bacitracin zinc) and it did not clear up. I went to the doctor and he put me on zithromax and said to use a hand cream. Two weeks later, it didn't get any better so I went back. He said maybe its a funal infection and told me to put Lamasil cream on it. It cleared up and was fine for about two months but now it has come back again. This time it itches. I have tried Lamasil, polymyxin B sulfate/bacitracin zinc, and cortizone cream (1%) and using all three at the same time (!) seems to help it more than using them individually BUT some days it looks like it is clearing up and then it bounces back to where it is looking bad again. I'm 57 years old. What should I be doing and if I take the oral Lamasil, what are my health risks?

Fungus infection is characterized by nail plate thickening, yellowing, separation and apparent debris in the area of separation. If you have none of these symptoms, then it may not be fungal, it could be bacterial. Medications for fungal infection do not clear up bacterial infection since they are two different 'things'. What color is the nail plate and do you have any nail plate separation?

Thank you, Marti, for responding so soon! Around the cuticle, including the sides, it is pink/red and a little puffy and the redness extending down to the lines just below my nail--between 1/4 and 1/2 inch. Like I said, I do have some itching. Some days it looks like it is clearing up and then it gets worse again. Also, 2-3 years ago, I did have a fungus infection in my thumb, same hand. The doctor said it was fungus and prescribed a prescription to use--kind of like a nail polish. After I ran out of it, I continued using lamasil off and on because it seemed like it was returning. My thumbs now don't match--the nail looks bigger on the thumb that had the fungas. Right now, I am having some redness/itching at the base of that nail ALSO! ALSO, my ring finger, same hand, is also having the same problems as my middle finger. A month after I fell off the bike, I think I poked it (maybe with something like a kitchen utensil--knife--I don't know but there was a little cut) and I think that caused some kind of infection. When I was put on the zithromax, the doctor was also treating that finger, as well. I didn't want to get too involved with my fingers but because you responded, I decided to tell you the whole story. Is it possible that I have two things going on and need to be on a prescription for both--something stronger than over the counter.
