What is the best way to keep nails filed down? The podiatrist used a machine, and though it did not cure my nails, it made them presentable enough to perhaps wear sandals this summer. Also, when using vinegar, are you suppossed to wet the nail with water first?

The podiatrist probably used a drill to thin down the nail plate; however, I do not recommend you do so at home. Go to you rlocal Sally Beauty Supply and purchase a 240 grit file in the nail department. Use the file to lightly thin the nail plate, but only once a month as you should only need to thin the existing damaged portion, not the new growth. Only use it if the nail plate begins to look thick again.
Do not apply water before the vinegar; apply it straight and a drop or two twice daily - always at night and always after your shower. It can take up to a full year to kill all the fungal spores because they live deep within the soft tissues of the nail bed as well as deep within the nail plate layers.
