We were refused authorization for an IVF procedure as Horizon blue cross has now set an age limit for its coverage and part of the procedure will be taking place after the 1st of april 2008 since when the changes in the coverage will come in play.
We would like to appeal and seek advice how best to it.

Unless you have a self-funded plan (meaning your employer pays the bills, not the carrier, they just administrate the plan) there's probably not much you can do as long as the insurance company gave adequate notice about the change to its policy. They just can't decide overnight to do something like that, and usually it has to be approved by the state, so chances are that's what happened.
Or also your employer may have wanted the age limit as part of the contract even if it's not a self-funded plan. Employers can pick different packages a lot of times to exclude certain procedures to lower premiums for the group (infertility, morbid obesity, etc.).
You could talk to your employer and ask for an exception but your employer may not be willing to do it just because if they do it for you, it sets precedent and then everyone down the line will want it as well. Which defeats the purpose of having it as an exclusion to begin with.
