going on for 3 weeks now. any suggestions? should i try a cream or something, and is it necessary to seek medical attention?

How have your bowel movements been? Have you had to strain?
It could be internal hemroids. If so then you can buy cream specifically for them.

Quit eating acidic things and spicy things. It helps a tonne to curb the infamation and itching.
Also wash area gently with a very mild cleanser. dab with hypoalergenic moisterizer (no perfume etc). Finish off with a cotton ball. This absorbs any slight leakage which contributes to the iching. I have had fissures and 'roids, this strategy works by far the best. I have used all the creams and ointments. Tell me how it goes.
