Hi,
I called Social Security to add some information to my medical file that is currently being reviewed and they told me that they requested information from me over 50 days ago and I have not responded. Well, lets see here, maybe that is because I never received it. The sad part is that they wanted proof of citizenship. Well, lets see, I was born in the U.S. and you have a copy of birth certificate that you requested last November. What else do you need? The person on the other line says, don't worry about, they won't hold your paper work up on account of this.
She then proceeds to tell me that I need to call this number and ask them if they have my file?????? Okay, I call and the number rings and rings.
Today, I get papers in the mail telling me to answer questionnaire Function Report?? What does this mean, does anyone know?
I was denied for SSD and I am working on an appeal. I think I have decided it is time to get a lawyer or I will never get anywhere.
How frustrating this system is.

Hi Pepper,
SS wants to know how you function in a day. They look at your answers to see what kind of doctors they may need to send you too.
Since filing for SS, I keep a daily log of what ails me along with a pain scale.

What type questions are on the questionairre? The only one I got is about my limitations,my points of view on what I feel I can and cant do. I answered in regards to my worst days.
