I know the simple precautions such as condoms and abstinence, but is there anything else that I can do to protect myself from being infected from a partner who is on antiviral meds?

There's not a lot else you can do. It really is best to avoid contact during the outbreaks. Even the act of putting on a condom carries a risk of transferring live virus. Once the sores have completely healed you are relatively safe until the next outbreak. The main risk then is that the contagious period may start shortly before the first indications appear.
