Once sexual activity has taken place on a virgin, the hymen will spilt. This is why virgins bleed after having intercourse for the first time. Once the hymen heals, a scar will be left so even if you stop having sex for a long time, the scar will remain and that will show to a doctor if you have had sex or not.

To add to what joansartiers said...
The hymen can also break or split due to physical activity...such as horseback riding, contact sports...not just from sexual intercourse.
It would be hard to determine if the hymen was ruptured because of sexual activity...so I think a virginity test would not be too accurate.

If you are asking about at physical exam to see if a woman still has a hymen, that is something else. If the hymen is completely intact, it is safe to assume the woman is a virgin. However, if they hymen is not intact, it is no indication the woman is not a virgin. It doesn't work both ways...
Having a torn or 'scarred' hymen, as joansartiers put it, does not tell a doctor if sex has taken place. It could have been caused by other things, like tampon use or masturbation.
