I Don't know the answers to your Question, other than that Morphine is strong and hard to come off of...
I Wanted to say that Ms contin is working REALLY well for me. Once I got used to it. It took about a week or more but now that my dose has been tweaked 3 times I am finally at a level that my pain is at a 4. Which is great.
It's working better than the Oxy at this time.
I take the MS Contin ER ( morphine sulphate) 3 times a day and I take morphine sulphate IR 2 times a day as needed. so far I haven't needed the B/T meds but 3 times in the past 7 days, so its really a GREAT pain reliever.
I would want to try Morphine before the bigee methadone. But that is just my personal opinion.
I am sorry to hear the oxycontin isn't keeping up with your pain. I haven't seen a post from you since you heard your GP had asked you to see a PM Dr. So I didn't know it wasn't working well..I always had a feeling your GP would do this since he was so far away and didn't have many PM patients.
I do think this is a good thing in a way because it was always an issue, and the drive was just to brutal and he was just to far away and not a specialist in the field of pain and that is what I think you really need. With winter upon us imagine that drive of 6 hours in the snow or a blizzard. Yikes.
All the best in your search for a Dr and getting the right pain relief. :D Slippery Slope.

morphine is a step 3 level drug, while oxycodone is a step 2 level drug.
Hope that helps

medications affect everyone differently so one may 'work' for one person and not another.

Thank you!
It seems too strong for me. But if my new PM doc wants me to try it Ill be open to whatever she suggests. the oxy works, I just need a stonger dose, I feel. honestly, I dont have any side effects, havent for three years, and it would be nice just to have my dose set accordingly. I remember my GP wanting to start me on something at one time, and I think this was it...
Thank you so much for the responses. Slip...You are the best with anyone who needs any info on anything.

Hey Izzy,
I'm not sure of the canadian steps being referred too, But most charts have Oxy about 50% stronger. They just don't make morphine product with apap so it's not commonly prescribed for milder pain in a step guides or sugested progression of drugs. Not every doc has a good compounding pharmy locally. Avinza is supposed to be the 24 hour version although many people devide their daily dose by two to maintain a smoother serum level. Kadian is the 12+ hour version where MSC is the 8 hour version. Personally I have swithced from equal doses of Kadian to MSC and felt no difference what so ever from the different releases, I adjusted the dosing schedule but used the same # of mgs a day.
The side efects of morphine are a little harder to get used to as far as sleepiness and constipation. Most prescribing info actually mentions giving it time to deminish. However everyone is different and and docs have meds they prefer and meds they avoid based on their own opinion and curent trends of abuse. Morphine is more abuse resistant as far as IV use so it doesn't have the apeal to people looking to misuse the med that way.
You do have to allow yourself some time to adjust to morphine. The drowsiness did get better, but the constipation and stomache problems seem to be a little tougher to get used to.
I've been seeing a group of docs for 7 years that simply don't use OxyC due to the negative press in VA. This has never prevented me from obtaining reasonable pain relief given spinal instabilty that still exists. I have reasonable expectations and don't expect to have 0 pain or complete relief. They do use SA oxy formulations for BT, just not the LA version.
I'm glad you finally found a PM professional and I hope together ya'll can work to find what works best for you, but I wouldn't be to quick to write off morphine in less than a couple weeks before you allow yourself to accomadate. This way you know you gave the med a fair chance.
The whole which is stronger discusion really isn't relavent since they have the abilty to create doses equal in strength using equiangesic guides. Given the right number of mgs of either med an equal dose can be achieved. 20 mgs of oxy is equal to 30 mgs of morphine on most equiangesic charts and from personal exp that's probably pretty close.
Good luck, Dave

Izzy,
What was your final dose of Oxycontin that you were on? Are you still on it with your new doc?

No, I havent gone yet...Im sorry if I implied that I had. My GP, the doc that has provided for me all of these years was going to try it at one time...But now that I think of it, I think it was Opana. I thought that it was morphine..Is it? Are many of you finding Opana works well?

MS Contin ( the Contin being the continuous effect) is generic form of LA Morhine. I find it a life saver. It works. I had terrible stomach side effects with the Oxycontin and even though I was only on it a SHORT time ( less than a month ) I had HORRIBL withdrawals from the oxy....I still have my perc for the SA for breakthrough but rarely have to use those. Have you tried any other long acting meds ? The Fentanyl patch didn't work for me as I was found to be a fast metaboliser ( I get over heated easlity and sweat caused too much to be released )
A good PM will listen to your symptoms, your life style and view your total history and see what has worked and what has not......I have read a lot of your previous posts and it looks like you ahve been on a lot of different meds. Your PM may want to give things a while to see if they work before changing or adjusting meds.
Your other Dr looked like he would be willing to give you whatever you asked for. I don't think a PM will be so easy to convince. If anything, they are more scruntinuzed by DEA than regular Doctors but they will work with legitimate pain patients to find a combination that will WORK

I'm taking MS contin LA and I really like it too. I originally took Kadian which is a brand name LA morphine, and that was like a god send. Like many of the others, I too find morphine to work very well and not have many side effects. I feel much more pain relief and much less "dopey" feeling with morphine than with Oxy. I am going back on either Kadian or Avinza in January when I have new insurance. I think it's definitely worth a try if your doc recommends it.

Hey Izzy, Opana, Is oxymorphone, It's in the same class as the other synthetics like Oxycodone, Hydrocodone and Hydromorphone. There hasn't been an oral version available prior to this one but its been available since 1915 as IV med used in hospitals and hospice. The IV version is called Numorphan and is 10 times more potent than IV morphine where the oral version is only 3 times more potent than oral morphine.
Just because a drug doesn't cross the broad brain barrier as effectively when given orally doesn't mean the right dose cant be found. I tried short acting opana for BT pain and it did work well. I would consider rotating with roxicodone for a month or two to take a break from oxy if there was a generic available. However with my medicare plan, It throughs me into the doughnut hole too quickly where my plan covers generics year round.
Take care, Dave
