why doesn't insurance cover foot surgery

It should if it the doctor justifies it is for pain and/or you are having trouble walking reasons and not cosmetic.

I agree... most plans cover it if it's for medical reasons.

Perhaps you have to try & fail with conventional methods first? I've read of that being the case.

Insurance will cover the cost of foot surgery if it is not considered cosmetic. For example if you have a bunion and you don't like the looks of it and it doesn'tcause you any problems they will not cover the cost of it.
