my donated kidney is at 3.5 creatinine what can be done to improve this?

There isn't really anything to be done to improve creatinine. Talk to your team (doc & dietitian) to make sure that you are putting the least amount of pressure on your kidney. You can decrease stress on it by lowering your consumption of animal protein and salt. Talk to them about what you are doing right and what you can improve.

As for when you need dialysis, that is a combination of numbers, swelling, and how you feel. Again, talk to your doc and see what he/she thinks. How are you feeling right now? Potassium levels are also another indicator of whether you need to start soon or not. Too high potassium can be dangerous.

As long as Creatinine is stable that is actually more important that the number. I myself have been around the 2 area .. and they said there have been people who had 4 creatinine and I was shocked. As long as it is stable is all that matters ... even though all of us would love to have a perfect creatinine ;)

Now on the other hand if you are getting signs of rejection then tell your transplant team right away!

One more comment. My creatininte was about 8 or 9 when I went on dialysis. I waited too long, but I could have gone on at about 6 or 7.
