Hello all

I just got a message from my doctor about my pathology report after having a colposcopy and 2 biopsies taken...which show moderate dysplasia. Originally this all I Believe began 3 years ago with my "problems" down there issues but I had an abnormal pap smear as LSIL in March 2009. The last pap smear I had was 3 years prior which showed normal. I have been seeing the ob-gyn on many occasions in this time for itching and irritation of my vulva/vagina/anal areas... I have had Bacterial infections which I take anti biotics for, then it will be a yeast infection where I will use my terazol cream and ketonconazole or diflucan tabs. I'm just sick of it. I received a call from the doctor about my abnormal pap and I scheduled an appointment for the colposcopy (i had to reschedule a few times due to certain circumstances) I Just had the colposcopy with the biopsies done last Friday. I don't even think the doctor looked with the microscope at the skin on the outside of the vagina which is what I am also concerned about. I sat in her office and she told me she sees mild dysplasia and that it usually always goes away on its own without treatment or damage to your cervix. I then asked her "What is Dysplasia?" She said "Dysplasia is abnormal cells" I then asked "And dysplasia is caused by?... HPV?" and she said "Yes it is almost always casued by HPV"

I am a little upset that they are dodging to answer Yes you have HPV. I am very concerned...

and now I have received a message saying that "Report shows moderate dysplasia. This is more involved than low grade or mild dysplasia. Good chance it will resolve on its own over time and smaller chance that it could progress to severe dysplasia. I recommend close follow up in young healthy adults. Try to eat right and exercise, do not smoke so your immune system can try and treat the lesion. Your other option is to proceed with a LEEP of the cervix, which involves cutting out this area. It can cause permanent damage to your cervix and has other risks associated with it although small."

What should my reply be? I am concerened that this crap is not going to clear up on its own because I think I have had this for 3 years now, with abnormal heavy vaginal discharge (they say it has nothingto do with it even though discharge comes from the cervix and the cervix is what has the abnormalities!), itching, itching in and around my anus (i am concerened of this too, I want a pap smear done of my anus).

Why do you think she didn't offer the cyrosurgery? I want whatever is up there removed but I'm just scared. I'm 22 years old, im white, I do smoke cigarettes (regularly since I was 18), I get really bad heartburn GERD, I can probably eat healthier than I do I can say...

I want to know what types of HPV were found in the biopsies, why aren't they telling me this information? Why aren't they looking for dysplasia on the outside of my vagina? I get irritation and itching and I believe they are related even though they tell me they are not.

I pledge to quit smoking today - I smoked my last cigarette last night and I am not going to buy another pack again.

Breal, first of all, congratulations on being a former smoker! Granted, it is only a few days, but the journey starts somewhere, right! :)

Secondly, I have several suggestions. I suggest you go back and request copies of your pathology reports, all the way back to your first pap (or the first pap that came back with a problem). The pathology reports tell what type of tissue was taken, if there was enough tissue for the tests, the specific names of the results, and generally gives a recommendation for treatment.

It sounds like you actually might have two different things going on. You have had a history of vaginal infections. Those infections could have been (or are) bacterial, yeast, or fungal. Each has a different treatment. Were you ever checked for bacterial vaginosis?

What type of doctor are you seeing? a ob/gyn? You might consider looking to see if there is a gynecologist (not an obstetrician-gynecologist) who specializes in vaginal infections. He/she might have a more effective way of treating the problems you are/were having.

Given your age, it is likely (if you are other wise healthy, and you remain a former smoker) that your body could fight the virus so it no longer causes problems in your body. Since you were a smoker, it may take your body longer than the generally assumed length of time for your body to do this (of several years). It is important that you get repeated rechecks to document the changes (or lack thereof) in the cervix. Generally, retesting every 3, 4, or 6 months is ordered, for several years.

When it comes to the itching of the vulva, vagina, and anus, I suggest you specifically ask your doctor about your concerns. Ask if he/she will do the colposcopic examination on those areas. The way I understand it, doctors don't normally look at those areas during a colposcopy, but that they can be done.

Finally, go to your doctors' appointments with written lists of questions. Leave room to write answers.
