I currently have my original SSD case pending at the Federal level, waiting since January 2009. Filed new claim in March 2009, I called my local SSD office and was told a new appointment Letter to see their DR. was mailed to me, I'm nervous as I went to their dr. before on my original claim and all he did was an EKG and asked a few Questions-this DR. had stated I can climb stairs, walk on uneven sidewalks etc., totally not true.
This time around I will make sure I tell him of my Disabilities that affect me when I do work. My DR. never filled out a Functional Report, stated they don't do that anymore.
Do you all think I should ask this DR. to please fill one out for me there, this was one of the items the ALJ judge reorted on his denial letter that I didn't have and this was one of the major reason why I feel I was denied.
I feel this may be my last chance to speak up, as I'm currently working very part-time 2 days-3-4 hours a day, 3-4 days off, then back to work, this is how it is right now, as my body can't tolerate working more days in a row. I am trying my best to work. Even with the short amount of hours there have been 2 days when I had to leave early due to stomach pain/diarreah.
Any advise would be apreciated.

they do too fill out a function report. both my primary and psychologist had to fill it out, as well as i how well i think i function. i work part time, too, bhtough only 5.5 hours a week (2.75 every sat and sun) good luck

they do fill out function reports,, i agree with Army on that,,,,
go to your doctor and ask for a copy of your medical records and read them and see what he (your doctor) wrote in them,,

Definitely get your primary to fill out a form. I was denied the first time and my original claim has been at the Fed appeals council since Jan 08. No word. In the meantime I filed a new claim in Jan 08 and was approved in July08. Old case still pending. I too work part-time no more than 24-30 hrs a month. Good luck and tell their Dr. all your problems I went to 2 of their dr"s and I think that is what helped me the second time around. Also they could see that I use a cane and had a hard time ambulating around.

Yes, it is very important to have all your Dr's to fill out the function reports , and any medical reports, changes in your health status , from the previous claim . This will support your claim of disability. Make certain you have the names and addressess of the medical dr's . You have been to in the last few months that you have seen that can fill out the forms. Every one of them that has seen you needs to fill one out. Psychological reports are important too. And any medications that you are taking too.
