Current Problems,
Started with intermittent left side around nipple area and upper part of chest pain that would range from a 4-8 on a 1-10 scale. Dull aching pain. Like a weight was put on my chest. Also high blood pressure the first couple of weeks but now is normal. This started June 28th 2003. Now I have somewhat of a constant 24hr type of weekness dull pain in that same area, along with occasional pain on right side, upper back & shoulder pain/discomfort, my legs have started feeling weird. Not swollen but slighty week. Have full range of motion. Seems to get worse as the day goes on. The latter part of the day before going to sleep is usually the worst part. See History Below For More Details. Any help would be greatly appreciated.

History-

28 years old.
5ft , 5in (Jun 28th, 202 lbs ) (Current weight 181 lbs).
No known family health history. Adopted.
No previous Medical Problems.
No Allergies. ( Except during this reacted bad to previcid , lexapro, Zoloft)
Never Smoked. ( Tried a cigar a couple of times made me sick)
Don't use any illegal drugs.
1994-Jan 2003 Drank 2-3 drinks every couple of days on average. More on weekends
1994-Jun 2003 Poor Diet. High Fat, High Carbs, Lot of fast food lunches.
Never drank Coffee.
Drank 5-6 soft drinks a day from Jan 2003-Jun 28th, 2003.
June 28th- current Only Drink Water, Better Diet. Bland Foods.

Tests Done-

EKG (numerous) - Normal
Ultrasound of heart – Normal
Heart Event Monitor for 2weeks - Normal
5 Chest X-rays - Normal
Thyroid Test- Normal 2x
Basic Treadmill Stress Test Stayed On 5min – Normal
Ultrasound of Gallbladder 1x - Normal
Stomach X-ray – Normal
Blood Tests- White Blood Cell, Red Blood Cell Count- Normal
Hepatitis Test – Negative
24 Hour Urine Test for Pheocromocytoma & Carcinoid Syndrome- Negative.
Cat scan Of Head And Upper Neck– Normal
Cat Scan from Chest to Pelvis (injected dye and made me drink contrast) – Normal
Cat Scan of Lungs- Check for Blood Clot In Lung
Barium Test For Acid Reflux- Positive For Acid Reflux, Neg For Stomach Ulcer.
Urine Test – 1x Came back with protein in urine. (said dehydrated?)
Mamography (stuck tube down throat and measured pressure) – Normal
X-ray of left shoulder - Normal

24hr Ph Test (Took while taking nexium, xanax because was still having symptoms while on nexium for 7 days), came back Normal. Was only able to stand 12hrs of test.

Symptoms- Left side chest pain around nipple radiating under left arm, tachycardia.
Tests Done – EKG, blood test, hepatitis test.
Diagnosis – Anxiety
Prescribed – Zoloft Starter Pack, Xanax .25mg 3x day
Referral- Cardiologist

July 1st 2003
Providence Northeast Hospital ER
Symptoms- Still Having Chest Pains
Tests Done – EKG, blood test
Diagnosis – Chest Wall Pain
Prescribed – Motrin
Referral – Said Should Go To Cardiologist And Do Stress Test

July 3rd 2003
Dr. Jani Spring Valley Family Practice
First Visit. New Doctor
Tests Done- Blood Test, Put on cardio event monitor
Prescribed – Stop taking Zoloft (see med notes), Take Xanax & Motrin ( see med notes)

July 5th 2003
Providence Northeast Hospital ER
Symptoms- Chest Pains, Had Severe Burning Sensation Episode Up Neck
(see notes Zoloft)
Tests Done – EKG, Cardio Blood Tests
Diagnosis – It’s still the chest wall pain keep taking motrin.

July 7th 2003
Dr Jani Spring Valley Family Practice
Symptoms – Severe Ringing In Ears.
Test Done- Hearing Test
Diagnosis – Said Motrin Caused Ringing
Prescribed- Stop Taking Motrin, Keep Taking Xanax
Notes: 3 Events From Event Monitor So Far Normal

July 9th 2003

Follow Up Visit For Test
Test Done- Ultrasound Of Heart- Normal
Prescribed – Stop Taking Xanax, Keep On Event Monitor

July 11th 2003
Palmetto Baptist Hospital ER
Symptoms- Shortness of breath, Left Side Chest Pain, Tachycardia, Tingling Sensation Top Right of Skull
Tests Done-EKG, Blood Tests, Cat Scan Of Head, Chest X Ray
Diagnosis – Don’t know, Said Not Lungs Or Heart Attack, No Brain Problems
Prescribed- Said Take 325mg Asprin A Day

July 16th 2003
Richland Memorial Hospital ER
Admitted To Hospital For 3 Days
Symptoms- Flushing, Chills, Sharp Left Side Chest Pain, Tachycardia
Tests Done – EKG, Blood Tests, Red / White Cell Blood Count, Urine Test, 24hr Urine Test for Pheocromocytoma / Carcinoid Syndrome, Psychologist Visit Test Anxiety, Stomach X Ray, Chest X Ray
Diagnosis- Don’t Know.
Prescribed – Stop Taking Asprin.

July 23rd 2003
9:00am Morning
Dr Jani Spring Valley Family Practice
Follow-up Visit Stress Test
Test Done – Basic Stress Test Stayed On 5 min – Normal,
Cardio Monitor Results - Normal
Diagnosis- Said to wait for 24hr urine test to come back. If still having problems will consider putting me on paxil.

Richland Memorial Hospital ER
Symptoms – Extreme Sharp Chest Pains In Center Of Chest
Tests Done- EKG, Cardio Blood Tests
Diagnosis – Said might be acid reflux follow up with GI people
Prescribed- Previcid (see med notes)

July 28th 2003
Palmetto Richland Memorial Internal Medicine Clinic
Follow Up Visit
Diagnosis- Said Anxiety No need for GI follow-up
Prescribed- Xanax .25mg day
Notes: Made Referral To Mental Health Clinic


July 30th 2003

Morning
Palmetto Richland Memorial Mental Health Clinic
Test Done- Saw Psychologist / Psychiatrist
Diagnosis- Don’t think it’s anxiety totally but may have some, don’t think its panic attacks. If anxiety would have to classify as atypical anxiety – general unknown.
Prescribed- Said to double the dose of Xanax to .5mg and if feel anxiety take 1mg.
Notes: Didn’t double dose of Xanax stayed on same dose.

Afternoon
Dr. Francis Goldstein (Internal Medicine)
New Doctor. Referred by good friend.
Tests Done- Did initial consult
Prescribed- Darvacet for chest pain

August 6th 2003
Palmetto Baptist Hospital ER
Symptoms- Severe Left Side Chest Pain, Nausea, Pain Down Left Arm, Back Pain
Test Done- EKG, Ultrasound of gallbladder
Diagnosis- Acid Reflux – Referred Pain to chest
Prescribed – Nexium 40mg 2x a day, Reglan 10mg 1x before dinner, Xanax .25mg 3x dy

August 16th 2003
Dr. Kimbrough GI Specialist
Referral From Dr. Goldstein
Tests Done- Upper Motility, 24hr PH Test – Both Normal
Diagnosis- Doesn’t think acid reflux is causing chest pain. Says his end is a dead end.

August 24th 2003
Lexington Medical Center ER
Symptoms- Chest Pains, Weakness, Some Sweating
Tests Done- Cat Scan of Lungs, Chest X Ray, Blood Tests, Urine Test, EKG
Diagnosis- Don’t know. Should follow-up with cardiologist & neurologist.
Prescribed- Tylox 5-500mg For Pain (see notes)

August 25th 2003
Dr. Hearon Columbia Cardiology
First Visit
Tests Done- EKG, Chest X Ray
Diagnosis- Don’t think it’s cardio related but will do nuclear stress test next week.
Prescribed- Nothing


August 26th 2003
Dr. Adams Columbia Neurology
First Visit
Tests Done- Shoulder X Ray, Tapped Both Arms, Tapped Both Legs, Watched me walk.
Pushed on arms and shoulders. (Total 15min)
Diagnosis- Said that he thinks it was because I stopped drinking 5 months ago and I’m recovering from alcohol and I should visit a support group. (Kept referring to his 28yr old son who is suffering from alcohol problems) Didn’t listen. Seemed to make up mind before exam.
Prescribed- Nothing

Reactions To Drugs Given So Far-

Drug History Jun 2003- Current

Xanax .25mg – Seemed to help with nerves slightly. No known bad effects

Zoloft starter pack – Took 3 doses. Caused severe burning sensations up back of neck.
Pain intensity went up with each dose

Motrin – After 6 days caused severe ringing in ears

Darvacet N 650– Provided almost no relief

Tylenol 500mg – Provided little relief

Lexapro 10mg- Took 1 dose. Caused severe burning sensations up spine and neck. Worse then the Zoloft

Asprin 325mg- Seemed to offer some relief.

Nitro Patch – Seemed to offer some relief. Given once in hospital.

Previcid 30mg – After 4 doses caused nausea and severe heaving / vomiting.

Nexium 40mg 2x a day- Seemed to offer some relief. No known bad effects

Reglan 10mg 1x a day – No noticeable difference in taking or not taking.
