I am terrifed of lifts, planes, the back of cars and any crowded place. I take a fan with me as i get really hot and I have tried relaxation but I get too anxious and panick. Please help!!!

I'm the same in lifts ever since I got stuck in a four person lift!
The worst place for me is the tube in London when it's crowded. I'm OK while the train keeps moving......but if it stops I get very panicky. And if the lights go out then aaargh!
I don't have any miracle cures but the medication and counselling I've had for general anxiety seem to have made it a lot better...
