When I was in seventh grade (13 years old - now 24), I noticed that whenever I moved my index finger, I could feel a small grinding inside the palm knuckle. Fast forward a few years to when I was in tenth grade, I was riding my bike on a dirt road and, every time I ran over a bump, my fingers would wiggle around and it was causing severe pain in my left index finger. It felt like there was a large quantity of something stuck inside my knuckle that hurt every time I moved that finger. I later noticed that, if I cracked that knuckle by pulling the finger out, it would end up feeling, once again, like there was something stuck inside that knuckle. But, if I made a fist and just used my thumb to pull down on that finger to crack the knuckle, it made the pain lessen for a while.

As of late, whenever I've been playing the guitar (I've been playing for almost 20 years), or when I'm picking up something large, or just doing anything that needs me to use pressure while keeping my finger flat, I'm getting that same "there's something inside my knuckle" kind of pain.

Is this arthritis? Is there something I can do about this? I don't want to stop playing the guitar and making music, literally every facet of my life revolves around the music I make and I can't give it up. I had to quit singing when I had surgery on my lung last year and that was hard enough on me, but I don't think I'd ever be able to give up my music.

Sorry you've been experiencing this for so long. There's really no way to know what it is without you paying the doc a visit. It doesn't sound exactly like arthritis to me. Have you ever had it x-rayed or had an MRI of it? I think you should get that done so that you know what you're dealing with first. Have you ever tried wearing a brace or splint on that finger? You might want to try one to see if it helps, wear it overnight so that it doesn't interfere with your daily activities. It shouldn't hurt to try that, but I'm not a doc, I'd definitely get a doc's opinion.

I am not a Dr for sure, but I think this can relate to OA. Unfortunately you playing guitar may increase the pain of your damaged finger. I didn't meet yet one guitar player who wouldn't complain about pain in his/her fingers and even wrists. Same as manicurists have "occupational hazard" and all develop OA in same joints and fingers or hairdressers get carpal tunnel syndrome due to their every day job.
I used to be very active in my gym and did a lot of lifting. I loved what it did to my muscles but I didn't like to feel what it did to my shoulders. Ignorant, I didn't want to go to the Dr to hear from him this is due to my lifting. Finally it got so bad, I couldn't write, forget lifting and had to see shoulder specialist. Yep, the verdict: either you stop lifting and do just cardio or you will need shoulders surgery. I stopped for some time, than it felt better and I started lifting again. We all know how it goes: OK, just a little, not too much.
Long story short: had 2 shoulder surgeries and had to stop lifting for the rest of my life. ;)

I hear you, none of us like to leave something we enjoy doing or doing it all our lives... Very sad, but when we age, everything changes in our body and we have to make choices either have worse problems and pain or make some adjustments.
