I would think some type of behavior modification program with rewards given when she does not engage in the fecal smearing might work or help. The trick would be being able to supervise her when she goes to the bathroom and then giving her a reward when she cleans herself properly and flushes the toilet. See if you can find small rewards she likes, i.e., stickers, a small plastic toy, a few M&M's or whatever she likes that you feel comfortable with and you can immediately provide after she goes to the bathroom and engages in the appropriate toileting behavior.
The toughest part is being able to catch her when she goes to the bathroom so you can supervise and immediately provide the reward.
