Post perineal/buttock pain,,mild bulging disk????

I would appreciate if anybody who knows more than me about back problems,,,can help me. I 've had pain in the perineum mostly,,for about 3 years, sometimes the pain is ffelt rectally, sorry for being so descriptive, ofteh times the pain is in the buttocks and upper thighs and back of the uuper legs, but mostly tingling , burning and just pain in perineum, buttock, inner front and back side if thighs. I've been wondering if this pain could be coming from my back or anything else. It all started suddenly 3 years ago,, I was exercising in the gym the same day ..but the pain started about 3 hours later whrn i was home..so dont really know what caused it. Its been really 3 years from hell ,,,
Had caudal steroid injection in the sacral area,,ddidnt do much,,,just numbed me for a couple of hours. Had 2 MRIs, nothing too obvious,,just 2 mildly bulging disks,,,says "mild diffuse bulging" at L3-4 and L5-S1. THe doctor doesnt believe too much that this mild bulge could cause me these perineal, scrotum,,rectal and buttock pains,,sometimes upper legs too. I am somewhat desperate.
Would really appreciate any comments and thoughts as to what this could be?
Also if i decided to do discogram ..does it have any negative effects,,Does it ever cause permanent complications,, can it actually make you worse on the long run?
What else other than the discogram can i do at this point to help come up with some diagnoses??

I wanted to ask just one more question in addition to what I wrote in my original message. Is it possible to feel pain in all those areas: perineum, i.e. around scrotum, buttocks, rectal pain, upper thighs pain...and not so much in the low back and still the problem to be from the low back, for example disk bulge? Is it possible for a disk bulging to cause pain mostly in the buttock, thighs, pelvic region and not in the low back?

Some of the areas your talking about could be from L5-Sl area. Sounds like maybe some more thorough testing needs to be done, but usually MRI's with contrast or at tops a myelogram will be done to diagnose a little better. Discogram would tell if the area is cause of problem but they will usually only do that if they are considering surgery pretty strongly.

I have some of the same pains. There is a website I will find and repost that you can hit body parts for affected areas of back.

Hi Jeff

Hi and welcome to the boards!

I am sorry that you are having a lot of pain,
and I hope you feel better.

This is my opinion and my opinion only. Yes, I do think that all the symptoms are comming from you back. I have some of your symptoms but not all of them, and mine is being caused from my L5-S1 disc.

As far as the discogram, I had one but don't remeber much. I was over sedated and wasn't aware of what was going on. It is suposse to be a diagnostic test to see if it re-creates your pain. I didn't have any permanent complications from it, but I can't speak for everyone. However, I can tell you that afterwards you are in pain for a couple of days. I just laid around and kept ice on my back.

Besides steroid injections, PT and traction, I don't know what else you can do besides....surgery, and that would be my very last option. If it has been going on for as long as 3 years, I think the pain is here to stay. You would be surprised how a "Mild Bulging Disc" can effect you as far as your pain!

I have disc tears at L4-L5 & L5-S1 with "MILD" herniation at L4-L5 and let me tell you, I am in pain! I have more pain in my hip, butt, and legs then I do my back. So your pain is most likely comming from your back. I had a hard time understanding that concept, but it is, what it is. I couldn't understand what my disc had to do with the pain that I had in other areas, until the doctor explained it to me.

I hope this helps you out. Please do remember, that this is only my opinion. Hopefully more people will come along to help you.

I have a very similar problem, and mine also arose several hours after an intense physical session. I have a protruding disc in my lumbar spine - between L4 and L5. The doctors were, and are, skeptical in my case, as well, as to perineal pain being caused by a bulging disc, but I would keep trying specialists until one believes you.

I have the same symptoms as you do. This came on after the gym with me also. My symptoms started at work the next day. I started feeling perineum, groin, buttocks and lower back pain. I have an MRI of my pelvis and lower back. My MRI of my pelvis back normal. My MRI of my lower back showed minor bulges I my lower back. I know it's been a long time since you have posted but i was hoping you figured out what was wrong with you and you could point me in the right direction. This has been going on with me for about a year and half now.

When someone has lower lumbar disc bulging, there can be some decompression or irritation of the spinal nerves. These nerves twist together in a bundle that form what is called the cauda equina down in the lowest level of the spine. These nerves then branch into the sciatic nerves which run down the back of each leg.

When a lumbar nerve is irritated or compressed, it is fairly common to have pain radiating down into the buttock, hip, groin, thigh, sometimes even running down the leg into the feet. This may be what is causing your pain symptoms.

Have you consulted with a spine specialist or are you being treated by your family doctor or PCP?

I have seen a few spine doctors. I have also had a MRI of my back. The MRI results state I have "broad base disc bulge which impinge upon the ventral aspect of the thecal sac" in the L5-S1. I have had a injection to the area but did not help my symptoms. I have had one Doctor agree that perineum pain could be a result of this type of bulge. All the other specialist did not feel it could cause this type of pain. I do have in the lower back, groin and hips pain. I have gone in all directions to fix this problem. I have had a MRI of my Pelvic area and a MRI of my right hip. The Pelvic MRI came back with a possible Sports Hernia and the Hip MRI came back with a possible Hip Impingement. With all these results I still feel my back is the center of all me pain. This thing came on in one day without any pre symptoms. I guess at this point I don't know which way to go.
