what do i do next???

i got in to see neuro suregeon in nyc(my 2nd opinion)

doesnt think surgery would help(which should be good news) not sure what or which herniation is causing pain, tingling, buzzing i am having since i was rear ended last july.
have multiple herniations / buldges in cervical and thoracic.

any suggestions - my nuero is currently giving me immipirine,siment and cymbalta

siment was helping stop the buzzing timgling but doesnt seem to help much any more, the cymbalta is getting me to sleep

any suggestions appreciated as i am out of doctor suggestions.

It would seem to me if they aren't sure what the pain generators are, that a discogram could give the answers they need.

I know you can't be eager to see yet another doctor, but I would do just that. Take your films with you....if you like the doctor and he doesn't mention it.. ask about the discogram. In truth, if all you have had is MRI, a myelogram and ct with contrast might be a better first step...then the disco.

If you are in this much pain, there is a reason; you will just have to keep shopping til you find a dr. with a sense of adventure who likes a challenge.

I was going to mention the discogram too. In addition to the physio and accu, you could go for epideral injections too. They did not work on my back, but did on my neck. So, you never know. Wishing you the best though, and get that second opinion!

what additional things would that show. both surgeons and my neuro said they believe the mris are more useful. what would it show?

2- discogram - can that be done on cervical and thoraic discs--i have multiple herniations

I was told if you get a good MRI, that other scans aren't needed. SO yeah, MRI is the better. BUT, if you have a discogram, then you will probably have the CT, but for a different diagnostic tool.
I had asked if I needed a myleogram and above is what my dr said.
You can have discograms on multiple levels, but I would assume they would be done at different times,well, maybe. So one day you would do cervical, another thoracic.

oh yeah, I had an EMG too, but that wasn't in the same field as the MRI or CT. The emg will check the status of your nerves etc..

We did the EMG first, because it shows whether or not there is an nerve(s) involved. If there is no nerve involved, then you take it from there.

I had mentioned a MRI with contrast, a discogram, and other tests to the doctor before my husband's surgery but he stated that it was obvious from the EMG that there was nerve damage and that fusion was the next step. I am wondering if we missed a step or two that would have saved him from fusion, but then we are always second quessing ourselves.

They will not do the disocgram for both areas on the same day,... are you kidding that would possibly save us a few$$$$$$$ But they say it is for our own good, that The cervical and lumbar or Thoracic, can not be combined. I asked if I could just do them together ...it sounded logical to me.

Get another opinion---from an orthopedic spine specialist! A neuro surgeon told me I wasn't in enough pain for surgery and he couldn't do anything for my legs. He also told me I could find someone willing to operate but I would likely be worse off. Then I went to an OSS who, after one look at my films, was ready to book me the day I met him. He went in posterior and did a laminectomy and fused me from L3-S1 with BMP on sponges. At seven months I'm doing great and no pain meds.

i have had an opinon from an ortho surg at beth isreal about 3 months ago but i was feeling good for 3 days and when i went in it was sort of why am i there if you seem to be getting better. i called him a few days later when the pain hit bad again but his nurse said thats theres too many discs involved so they were not sure what they should work on and since its all over(arms and legs it sounded more neuoro related.. just so you know i currently have pain in my thorasic area- like a knife or broken glass, buzzing/vibrations in arms and legs along with burning mostly in left arm. this all started the day i was hit from behind and slowly got worse with some good days that make me think its going away.

the neruo surgion i saw was my 2nd opion from a neuro seurg - the first was after my nuero got my mris back a month after the accident. he told me i'd be fine in 3 months. when i wasnt and i called back after 4 months he said if you want me to operate i will but wont help arm pain. -He also said i have 4 or 5 bad discs so it would be a huge surgery-

i am totally confused since the 2nd opion nero serg said there are no exercises or anything i should do except go back tpo mty nero and get her to change medications to help with pain.

By the way my emg was basically fine. it did so potential nerve problems in lumbar but lumbar mri was fine.

when i was rearend my body went forward and then my head snapped back. i felt something weird in th en eck happen and felt like a knife going through my thorasic -the same area that hurts now. I know all this crap is coming from that and believe neck stuff is causing arm tingling pain and thoracic is affecting legs and back but no one seems to corralate that.

i am lost confused and getting depressed. i would love a caring doctor would can tell me what i need to try and do so i can do it whether it be pt again, appropriate drugs or other tests. I will def ask about discgram.

Another two cents:
Okay-I saw an ortho first regarding my neck and back pain. He said I was too young and he'd do no surgery to help me. He told me to go and have epiderals in my neck b/c my NECK was the issue of all my problems. He said if I did not go get the injections, I should leave his office then and there b/c there was nothing he could do for me. He did not offer anything to me and did not say he ever wanted me to see him again. I left crying and my husband wanted to punch his lights out. So, I don't think one dr is better than another-I keep saying that but I have dealt with two orthos and one NS and of the three the NS is the best and the nicest. BUt, that doesn't mean I would never see an ortho again. I think it is the doctor, not the profession. I also wonder if age has something to do with it too. BOth Orthos were older and seemed bored and tired of us back people like we are a waste of their time and push us around. The NS sat and listened, asked if he answered all my questions, took his time, smiled at me, and even walked me to the check out counter, and does this every time. He always shakes my hand with a smile and pats me on the back saying" I am so happy you are feeling well" or whatever. He is under 40 and I don't think he has been doing this long enough to be annoyed with us. I really really think that is the problem. I don't think we should say ORTHO is better than NS or the other way around. The kind of person he or she is makes the dr.
NOW, I had the MRI first. SHowed the disc issues(cervical and lumbar). The xray showed my arthritis (neck and back)(MRI too) and loss of neck curve. A week before my injections (PM doc disagreed with the Ortho and was shocked to do so b/c he knows this dr very well-he said it looked like my neck caused my neck pain and my low back caused my back pain) I had the EMG and nerve conduction. The emg showed a pinched nerve relating to my C-spine. No other nerve problems. I had traction with the steroid pack. Then, I had my injections. One into the low back and a transforaminal one in the neck (two injections, one on each side). After a month, low back was not getting better, but my pinched nerve symptoms went away (tingling, arm pain, hand weakness etc...). I saw my NS and he felt that there was no surgery that would really help my neck. Most people he does surgery on still have pain. BUT, he did feel I may be a good candidate for the lumbar fusion. Then I had the discogram which showed the two discs were bad. We then decided to put my neck on hold and go ahead with the lumbar fusion of L4-S1. I am happy to report it went well and I am very glad I did it. The future is uncertain for my neck though. There is no sure surgical treatment for spondylosis-which I don't really want anyway. So, I have to wait until March to discuss my neck further, and see how well my back is doing in the healing process.
I hope some of this has helped a little bit. There aren't many of us here with neck issues-and so far none of us have been given a definitive answer on how to treat it. But, I will be one of the first to mention it if I do get some answers and relief.

you are a wonderful person,
i was in a fog thursday after walking out of the neros surg office and him saying no excerises i can do to help, and no suggestions other than change meds or potentially shots.

do u do any neck exercises for pain or stiffness that works.

I had some exercises that Physical Therapy gave me, and they worked for a bit but stupid me quit doing them when everything else seemed to avalanche with the arm stuff and low back. I only went for two weeks of PT on my neck and that was back in August. I am looking forward to going back. Massage does wonders-they know how to work the muscles to relax and release the tension. Ultrasound, heat and ice are great too. Tractions is supposed to be a great thing for the neck, so if it won't hurt my low back with the fusion and all, I will see if that may help. Exercises are hard to explain; let's see. When sitting straight up, look down to your chest, press gently on the back of your head to push a little further til you feel your muscles pull. HOld that for ten seconds. Do that looking down, back and side to side. Chin tucks are good too. Stand against a wall, with a pillow behind your head and tuck your chin to your chest. Now, be sure to tuck BACK, meaning to flatten your neck against the wall and pillow, don't bend it down. Do that ten times, rest and reps of three. There were some others, but I dont' remember them, sorry. HOpe these help, maybe in the meantime you can see PT again-it can help or atleast get you some relief in the interum.
Now, I just realized-we are talking about the neck, right?? If not, nothing really worked for my low back. Best wishes!
