What could this be?

Every morning when I wake up now, if I lay on my back, when I exhale I have a throbbing pain along my spine. It's located mid to upper back, and the throbbing is the beat of my heart. I only feel it when I exhale and if I'm on my back. If I switch to another position I don't feel it.
