Wanna play Dr?

We have the results of dh's MRI from last week. It has changed a good bit since his first one last May. He has an appt with the neuro this afternoon to decide the next step which is leading to a multi-level lumbar fusion. I'm here to ask the 'real experts' what we should be asking the dr. I hope this won't be too long - I guess I want to hear from anyone who has treatment for any of these findings.

L1-2 - loss of disc sig, minimal bulge
L2-3 - severe disc narrowing, vacuum disc, sclerotic end plates/osteophytes, extensive lumbar degeneration, slight retrolisthesis (spondylolisthesis), small bulge, hypertrophied facet joints, right L2 nerve root may be contacted by disc
L3-4 - mild loss of disc sig, minimal bulge, hypertrophic facet/ligamentum flavum w/ slight encroachment on central canal & foramina w/out mass effect on nerve roots
L4-5 - disc narrowing, diffuse disc bulge, more hypertrophic changes, mild right and severe left foraminal stenosis, cyst on left facet joint
L5-S1 - loss of disc height, diffuse disc bulge, hypertrophy, conjoined left S1-2 nerve root

Impression - multilevel DDD, L2 disc impinging on right L2 nerve root. Moderate to severe stenosis.

So, what is the biggest contributing factor that you can figure that is causing the chronic pain? I think a 'normal' MRI would show some DDD but how bad do you think this is? Be honest, please. He also has some significant problems with the cervical area in addition to two very bad knees.

I had an orthopedic spine specialist. Ask if he has a spine fellowship. If not find one who does. Mine was so cutting-edge that I had an off-label procedure with BMP.

Thank you both! I've got the questions ready for this afternoon. My husband has tried the noninvasive treatments - PT and injections - no relief. He is seeing a neurosurgeon for his back but an ortho for his knees. The orthopedic surgeon is the one that recommended the neuro. This neuro did a microdiscectomy back in July and dh did fine with it but the other problems have finally taken their toll.

Apparently ADR is not an option due to the multi levels needed. As far as restrictions go, back or not, dh has physical limitations and was originally placed on LTD due to his knees. I guess what I'm saying is that he can't do much anyhow. We're just now at the point where the desperation for pain relief is priority. I do know that his neuro uses cadaver bone. Now I need to look up 'BMP'.

Thank you both again for taking the time to read through this mess.
