How do I find a quality PT or trainer for my back?

Hi. I've had back pain for two years and in January it hit me so bad that I spent the better part of 2 weeks flat on the floor. The orthopedic surgeon is prescribing physical therapy again - this will be the 4th round. I'm still willing to believe, but I want to work with a personal trainer or physical therapist who can really help me.

Can anybody give me guidance on how to find a truly quality person in Seattle who can help me get back to kayaking and hiking and running?
