Fusion the next step?

I have set a date (Nov 27) for my back fusion of L4-L5(TLIF). MRI shows bone on bone of L4-L5 and Arthritis in the facet joints of L5-S1. With a discogram pain of 10 at both levels.

My past treaments as best that I can remember.
1. About three different PT from 2004 - 2008
2. Some massage.
3. To many chiropratic adjustments to count.
I was seeing one Dr. for about a year. Started with 3 times a week, to 2 times a week, etc. Once a month etc. = Some relief 4. Acupuncture
5. Epidurals shots with no pain relief. (2004)
6. RFA radio frequency, no pain relief.(2005)

September/ October 2012 Went to three different spine surgeons with current x-rays,MRI,discogram, & CT.
Two of the three said I needed to have a two level fusion PLIF/ALIF 360
One said just L4-L5 TLIF. He felt that L5-S1 disc still looked good. And could get a few or more years out of it before it would need to be done?

So would most of you agree with all of this? Or have any other ideas for me to look into.

I have posted a few times already, but this is by far the most information I have given.

Re: Fusion the next step?

Are you an avid golfer and do you hope to keep playing after surgery?

Re: Fusion the next step?

Yes I do enjoy playing golf. The last time I played was July with the most pain I have had playing. No plans of playing again until my back is fixed. Doctor did say I should be able to play by mid summer 2013.
