Tailbone aches/pain and Butt Piriformis and/or Sciatic aches.

Hi all,
I was not sure what sub-thread to put this in, so I felt maybe this 'back problem' thread would do.

I am a 29 year old male.

I can't believe it has nearly been a year already, but I remember waking up last summer with an achey tailbone. I am 98% sure I did not injure it. I wouldn't go as far as saying it is painful, but it aches, and it can hurt when it gets inflammed from sitting too long. Part of my problem is that I have had the same job for many years where I sit on my *** A LOT. Lots of *** sitting! And I am a skinny guy, so my bony butt has no cushion!

I went to my physician, and he said to work on my posture, and excercise more. So I did. He also prescribed me anti-inflammatory pills. Neither of this worked.

A few months ago, I noticed I also had a bump on my tailbone. If I kind of dig in there, i can feel it and move it around. It is Not an pilidonal cyst. Because it is not above the skin, it is below and actually feels like it is on the bone itself.

What also sucks, is that my right buttocks aches too! I am guessing this is associated with my tailbone, but maybe not. I have no clue if it is nerve, muscle, or bone, but it seems either it is Piriformis Muscle or Sciatic Nerve or both...

Having a job where I sit a lot, this can really be annnnoyyying! I have tried donut butt cushions, but to me, it makes it worse, and all it does is transfer the pain to other parts of my back and butt. I have to constantly wiggle around in my chair and change positions to try and alleviate the discomfort.

So I went to the my physician again a few months ago and he referred me to a chiropractor. I am hoping they will do X-ray and all that good stuff. Unfortunately they were so backed up, I am barely getting into my appointment in May!

I guess I am just wondering if anyone has had similar issues before? Or think of what it could be? I am just a bit of a hypochondriac, so I always have a worry. Any thoughts or ideas I would appreciate... Thanks!

I would suggest making an appointment with a spine specialist before seeing the chiropractor. This could be either an orthopedic spine surgeon or a neurosurgeon whose practice is limited to the neck and back.

You may need to have a MRI or CT scan to see if there is a cyst or some sort of soft tissue problem. Since you have had this for over a year I think it is important to get an accurate diagnosis and rule in or rule out a potential spine issue. A chiropractor will not be able to do this. His knowledge is limited by his training.

Oops, my mistake. I meant to say orthopedic, not chiropractor. I will indeed be seeing an orthopedic.

In addition to your tailbone hurting, you may be having sacroiliac joint pain. Since this has been ongoing for a year, you definitely should have xrays of SI joints and lumbar spine. You could have a pilonidal cyst, as those are deep, under the skin and that may be what you can move around. They can get inflamed from too much sitting, but can also get infected. Close inspection shows a tiny opening, sometimes in a dimple, like an enlarged pore, right over the coccyx. As far as the SI joints, in your age and being male, ankylosing spondylitis should be at least considered. It is in the family of rheumatologic disorders, and typically starts in the SI joints with inflammation, pain, stiffness. It is often found in family members of those with autoimmune disorders or a family history of AS. The earlier it is identified and treated, the better. A rheumatologist diagnoses and treats that disorder. Hope you get some answers and relief soon!

I finally saw an ortho after months of waiting...

X-rays were taken. He said my bones look 'great'. He said the ache/pain in the tailbone may be caused because my tailbone is straight, with a little curve at the point. It doesn't totally curve like most tailbones. It isn't necessarily uncommon, but this may be one of the reasons why it aches, and since I sit a lot, it easily gets inflamed. He did feel the bump that I have. He said it could be a cyst on tissue or muscle, but doesn't appear to be on the bone according to X-ray. He was not so sure about my right buttocks ache and whether it was related or not. He said it could be the sciatic nerve. He mentioned that I should not be too concerned...

For now, he recommended I do some physical therapy and do some stretches. And I will follow up in 6 weeks. He said he will probably have an ultrasound performed that time around to try to pinpoint where and what the bump is. I am not sure why he just didn't do it then, but he said it may take a couple visits, but we'll "figure it out."

Glad you got to see a doctor and have received some information. Was this an orthopedic spine surgeon you saw?
