Most doctors here in the US believe counts less than 30 should be treated. I don't know what they think there, but you might need to be more assertive if you want to treat now.
ITP treatments are super-expensive except for prednisone, which is dirt cheap. I never had to take more than 40 mgs a day, and that was only at the beginning of treatment when my count was just 4. Once it got up to around 65, I was able to maintain that with just 10 mgs of pred a day. It would be worth a try for you, since you have fatigue affecting your well-being.
High doses of prednisone create their own major problems... Side effects galore, some quite serious. So you'll have to decide if those would be worth it to alleviate the fatigue. Low doses like I took have very few side effects, though.
