For more than two years now, I have had constant loose stools with virtually every BM. These stools are not quite like diarrhea, but close. It is quite unusual to have a solid stool. I have 1-3 BMs per day. I find myself needed to return to the bathroom and re-wipe several times after a movement due to severe leakage. Also, virtually every time I eat a meal, within 5-15 minutes, I must go to the bathroom to have a BM.
I also have frequent gas.
I eat a normal diet, with at least 1-2 servings of leafy vegetables per day.
I am 47 years old, male, Caucasian.
What is wrong with me? What can I do to overcome this problem?

You need to get more water soluble fiber in your diet. It is in grains, root veggies, beans like limas, pinto anything that absorbs water/ fluid.
It will bulk up your stool.
Also psyllium is good.
It also sounds like you are intolerant to something you are eating that is causing the imediate BM after eating. Coffee is a bad thing for many people!!
Thanks! I have not had coffee for three days now, and it is getting better. Still not where I want to be, but now I can experiment with other foods as well.
FrankJohnson... Harry has a valid point. He noted you should increase your soluable fiber intake. It will help bulk up your stools and keep you more regular. Maintain this for a while to see if it changes your situation. Good luck.
FrankJohnson your story was my story. It's like I wrote that 4 years ago.Went to the doc back then and he said I had too much liquids in my body and that I needed more fiber,so he recommended Metamucil. So every night I have a glass and most of my problems ara gone.I use 1/2 cup of water and 1 1/4 heaping tablespoon of powder. It really works.
