external vs. internal?

Hi,
I've had a hemorrhoid for a while now (actually more than a year... I know, horrible, I've been putting it off). I keep telling myself that when I actually get around to using the cream I bought it will go away, but that's probably not going to happen...
Anyways, I was wondering, how can you tell if it's an internal or external hemorrhoid? I'm applying to volunteer abroad and they have some health restrictions, one being that you can't have internal hemorrhoids. I think what I have is an internal one... what happens if I do? How do I get rid of it? Will the cream get rid of it or do I have to have surgery or something??? eek...
