Can't hold it in

I can't hold it in when I need to go to the restroom. Both #1 and #2. Today this morning I woke up, someone was using the bathroom and I went inside my pants while I was in my room because I couldn't hold it in. This has been going on for a while now. I'm afraid to tell my doctors. What could this be?
