Red face Need advice for fissures 6 weeks after Hemorrhoidectomy!!

I had 3 internal hemroids removed with ligasure device 6 weeks ago and have had continual bleeding and pain with bowel movements. Now I am told I have 2 fissures, not sure when they occurred because there has never been any relief from the pain and bleeding in the past 6 weeks.

I am very scared this will never go away without surgery. Have been trying annointment for 2 weeks now to help with pressure/pain but I am still having a lot of bleeding.

I have been taking metamucil and miralax (when needed) faithfully since the original surgery.

I am currently looking for a specialist with knowledge on treating with Botox. Does this work? Are there better options?

Help...I would like to hear from someone with any experience with this. Thanks.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Sounds like you need to get in with the specialist or general surgeon asap. If the fissure gets worse - you will need surgery asap due to the pain which is totally different than the hemorrhoids.

Unfortunately, sometimes, the ligasure doesn't work. A close friend of mine started out with the same problem you did and had the ligasure - she had the same problems after surgery so they ended up having to do the old-fashion surgery. She was in pain about 2 weeks but had heavy pain meds, but had no more symptoms after that. I would get in as soon as possible so you can hopefully stop suffering. I know it's embarrassing but will only get worse. You also may want to just f/u with your first surgeon right now.

Hope you get to feeling better.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Thanks for the quick reply. I have a call into the specialist office, will definitely try to get in ASAP. My original surgeon has recommended I see someone else so I will be more comfortable with any new decisions since this didn't go as planned.

It is good to hear from others who have knowledge of this problem.

thanks..

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Heya! Let us know what your doc says......I too have a small fissure or two about 10 weeks post op. My doc cauterized one of them two weeks ago (which took care of the pain almost immediately), but it is acting up again. My next appointment is Monday, thank god.

There is no blood, just a flash of searing pain when I go potty that is over rather quickly. Compared to how i felt ten weeks ago, it's a cakewalk.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Hi Happybottom,

Love the name!

Good to hear from you. I definitely have less pain than 6 weeks ago, but am worried it is becoming a chronic issure. Still seems like a lot of bleeding and pain, very discouraging, also a very tight feeling all over pelvic region. Maybe it is not unusual if you are also experiencing it at 10 weeks. Hope your Dr's visit is helpful, please let me know how it goes.

I tried to get ahold of Dr's Friday but they were closed because of good Friday. Guess I need a referral and have to be approved as a patient to get into a very well known clinic, that is my goal next week. Not sure what I'll do if they don't accept me. I do have a referral to another specialist but they do not do the botox injections there.

Take care...

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Heya! My doctor cauterized with silver nitrate again, and said if I am still having issues next week that she will prescribe a special cream for me that will aid in healing. i don't recall what it is, but i thinkg she said it helps increase blood floow down there and speeds the healing---I will get back to you on that. I did some research online and only 5% of "fissure people" have to deal with them constantly post-op. So the odds are in our favor!
But it is still a royal pain in the butt right now. I am using my rectogel cream (lidocaine with aloe vera) and it takes most of the burning away. Like i said, compared to the first few days post surgery and that very first bowel movement (oh my god) it is pretty minor---but still incredibly annoying. I think I might start up the stool softeners and bulking agents again...I probably should have stayed on them longer than I did.

Any news from your doc??
Hope you feel better soon!!!

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Good to hear from you. I couldn't get an appointment until April 7, and with another specialist April 21. Seems like a long....wait....

I am using a specially made ointment, not sure which ingredient but it is supposed to increase blood flow and reduce pressure. Probably what your Doc told you about. Have been using it for almost 3 weeks now. It may help some, but the problem is still there. The nurse said it can take 6 weeks to heal. Hoping that will work, but it still has a way to go.

Do you think the cauterizing with silver nitrate helps? I will see if the Dr. recommends it. Hope yours is healing so you won't have to deal with it much longer. It seems like a long ordeal that will never go away!! Are you down to one fissure?

Probably good to keep taking fiber supplements. I am having 1-2 doses of metamucil per day. I felt the miralax helped me more than stool softners, so I've been taking that alot of days.

Hope you are feeling better. Take care.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Hi krazijess,
Thanks for the reply. I know what you mean about regret...I totally regret the hemroid surgery, no pain before, now tons of it....still 2 months later., though it is better than a month ago.

Did you try any other treatment before botox? Can you tell if things feel more "relaxed" now after botox? I seem to tense up after BM's and have alot of pain for many hours.

I have 2 appointments coming up, next week with Doc that doesn't do botox. Another later in the month where botox is a possiblity.

Have you tried the annointments that are supposed to relieve the pressure to allow healing? Botox sounds like it may be better at doing this.

How long have you been dealing with this "issue" before trying botox?

Hope you are feeling better very soon....

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

I didn't have Botox for my fissure, I had traditional surgery for it. At the same time I had an internal (unexpected) hemorrhoid removed. It was the most painful thing I've ever experienced (and I have gone through labor/delivery). The first 3 days afterwards I was in excruciating pain and the painkillers didn't even help. (In hindsight, I should have gone back to the hospital for stronger meds but that was my mistake)

All that said, it was worth it for me. I haven't had issues with any fissures since the procedures. It has been 3 years this spring. I did have some anal cramping here and there for a couple of months afterwards and every once in a while I'll get a couple. but otherwise that part of my body is doing pretty well.

Good luck in whatever you decide and hang in there. I know how bad dealing with these types of issues can be.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Hi Josey,
Thanks for the encouragement. It's good to hear from others who have had a good outcome from their procedures.

I am anxious to have this resolved, hoping that the Dr's appt can give some good advice and resolution of this very annoying problem. It seems to take over your life. It is a very painful issue, although the hemrroid surgery was way worse. I agree that the pain seemed to be worse than childbirth and other surgeries I've had. Wish I would've been warned of the consequences. Don't think I would have agreed to this.

Happy that you are well. Take care.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

Well, I saw the specialist today. Very disappointing. It's been 2 months since the regretted hemorroid surgery. The Dr. just told me that everyone heals differently, and I am still healing. I guess bleeding and hours of pain after 2 months is not unusual?!!?? Come back in 6 weeks and we will see how you are doing. You can continue using the ointment - he didn't make this sound all that important. He also said we don't want to do any more surgery yet. I absolutely agree! Guess I just need to be patient.

I am wondering what happened to you'll be fine and ready for work the following week after surgery (that came from another Dr's office).

I have another appointment scheduled April 21 with a different specialist several hundred miles away. Not sure if it is worth the trip, though.

Anyone have any advice?

Hi Jess,
Hope you are feeling better and the surgery is a success. Wondered what happened to you.

I am not sure what to do, I have an appointment coming up April 21. Hoping I feel more comfortable with this new Dr., otherwise I don't know what I'll do.

Let me know how it is going. Wishing you a speedy recovery.

Re: Need advice for fissures 6 weeks after Hemorrhoidectomy!!

I am SO Discouraged today. After nearly 10 weeks, things don't seem any better than they were a month ago. I am really regretting having this surgery...and angry at the surgeon for not giving me any clue that this could happen. I had No pain before this surgery, only bleeding. This surgery really didn't seem necessary. I was very uninformed.

Does anyone have any idea how long to expect this to last? Does it ever get better? Right now it seems like things will NEVER get better. I am still having bright red bleeding and pain with every bowel movement. The pain can last for hours. I also have discomfort alot of the day although I usually feel better when I wake up in the morning until the dreaded bowel movement.

I take metamucil several times a day and also take miralax almost daily to keep things moving.

Any encouragement would be appreciated.

If u live around Northern Virginia/Dc u can come and see Dr Seid ..he is very good...very little pain, u berly need painkillers...at least so far.
he did gave me Volume and percosets for pain and relaxing my muscles...It was less painfull than Botox injection...Ask ur Dr how and where exactly he is gono cut the sphincter...this is very important and main reason ppl get incontinence
