Piles come and go. Use the cream and Lactulose until the soreness clears up. If you want to limit the frequency of further flare-ups change your diet to keep your stools soft as constipation is a killer for bringing on another dose.
There is plenty of information on this forum from the 'piles r us' sufferers to help you with a diet. People use different methods but basically it's as much water as you can comfortably drink, include plenty of fiber (both soluble and insoluble), fresh fruit and veg and I use olive oil with each meal as a natural stool softener because it's not advisable to take laxatives regularly.

Oh by the way the wet arse is part of the cure. You will dry up when the piles subside. Get used to it as it's part of the deal. Avoid any medic's suggestion for rubber banding. It's unnecessary if you keep your diet right.
