I've read a lot of the posts here, but still have some questions. Let me start at the beginning. I had my sigmoid colon removed in august 2009, due to stragulation by adhesions. At that time my surgeon noted I had internal tears in my rectum. He dialated my anus to aid in healing. Since the surgery last year I have had various degrees of anal bleeding, pain, itching, etc. I thought it was a hemorrhoid. Turns out that the tear inside came outside. He sent me to a surgeon. The surgeon said the tear inside was 2.5 cm and he cleaned it out, then cut good tissue and flapped it over the tear to heal. Well, 3 months later, I'm still bleeding and some clots, and heavy mucus. Infection set in. 1 month on antibiotics helped with the internal healing, but about 1-1.5 cm external fissure still existed. He said he had to operate again, since it did not heal for so long. Well, here we are 3 months later again, and I have to wear a sanitary napkin because of the bleeding. I have to change it 4-5 times a day. I am not having my period anymore, so I know its anal bleeding. I have tried everything from creams, soaks, everything, but nothing is working. My bowels are regular, soft and formed as I take a dietary supplement (fiber) and it works great. Lots of water also. One odd thing is this....at times I feel like I have to push hard, almost like bearing down, like a spasm, but no bowel movement. Its like my rectum thinks there is something there when nothing.
I know this is odd, but I am at my wits end. I am an American living in the Caribbean, and it is difficult to get American Medical Advice from so far away. I'm seeing my MD tomorrow to get a referral to another surgeon (protocologist maybe??) to see what the heck is going on. Any advise?

Some people seldom have to push and because of a past condition have had to learn not to push at all. What foods a person chooses may help also. I have heard many a person say "I have used fiber and I use water and I have hard or constipated stools" but that is not true with all of them. To have to push hard in my opinion could make a the pressure at that end a bit too much. It seems the muscles should be doing most of the work for you. What number of B.M.s a day do you have?
