Your issues, minus the back stuff, sound like mine.
I had a earth shattering bowel movement almost a year ago and have been in a bad way ever since. For me, it started with working out. One day after being at the gym I sat down with a good book and started reading then bam! I felt what I describe as a tearing sensation. I ended up, and you may as well, with an anal fissure.
When mine acted up originally it would hurt to have a movement and there would be blood. But after about a week, it would go away. Some time later it would come back, same deal for about a week. Lately, after another painful movement the fissure seemed to not be going away. I also have a lump after I poo. Feels, to me, like I'm sitting on a bruise. With fissures come piles and I figured this to be a pile sitting over top of the fissure. My fissure seems to have healed, at least mostly, but the pile remains. It may be a hemorrhoid or it may still be the fissure acting up.
Act now! Go see a specialist. It's MORE than worth it. I was treated with Dilitizapim (spelling?) originally and that really helped at first. The pile (Swelling) would not be there after a movement. However, I experienced a side effect (your results may vary) of insanely itchy ******** and I couldn't take it. Now I'm on nitroglycerin and that doesn't seem to affect the lump but other than that I'm ok. If it is the start of a fissure, you want it to heal sooner rather than later. Fissures are made worse by spasms. If something in your lower back is somehow causing your anus to spasm as well, it could be bad news and will make things worse. Go Go Go to the colo-rectal surgeon! You have nothing to lose and your life to gain back.
