I have this same issue....sometimes it accompanies a headache....sometimes there it does not. Sometimes, I do not feel stressed at all. I just wake up and the vein is already pulsating visibly. The shows on both sides for me, but it is much more pronounced on the right side and you can see it pulsating without having to look closely. I think what concerns me is that when it's at its worst, I feel a squeezing sensation against my temples (kind of like my head is being squeezed). My blood pressure usually spikes as well (mostly the bottom number). When I feel this squeezing against my temples and the vein gets large and starts pulsating, my blood pressure will go from 120/75 to 135/100. Sometimes the top number will still be at 120-125, but the bottom number will still be approaching 100.
I've suffered from migraines since college, but this does not seem to be directly associated with the migraines (since I am having this issue even when I am not having migraines).
As a side note, I do suffer from anxiety disorder. I used to have panic attacks almost daily. I used to take Lexapro for it, but stopped about a year ago. I have not had a panic attack since getting off of the medicine though.
