Slr is exactly right! Tumours aren't always cancerous. A malignant tumour is a cancerous one. Okay, so, if your friend had bladder carcinoma and there are now lesions on the outside of the liver, yes, that means that it metastasized because the chances of having a primary on the liver AND a primary in the bladder is very slim.
Another note, you know that this is cancerous, because metastasis (and invasion) are properties of cancerous tumours--not non-cancerous ones.
