I don't think that we can ever actually pinpoint one particular thing that 'causes' any type of cancer..there are so many things that could contribute that we never know that we even have contact with. However...smoking...is an obvious contributor as well as something that is preventible. I'm sure what your doctor was referring to as your smoking 'causing' your cancer is the fact that smoking inhibits the body and cells to do what it does naturally which is fight off disease and infection because of the toxins in the cigarretes and the effects on the oxygen levels that the cells are able to get.
So I think for someone to think that their smoking had 'nothing' to do with their cancer forming is naive or possibly in denial that their addiction contributed to their condition.
