Second Pap After Leep Procedure came back ASUCS & HPV

I have a few questions. first I would like to introduce myself. I am a 33 yr. old single mom with a wonderful 5yr. old son. My story is April 2007 I had to have a leep procedure done and it came back as moderate to severe when I went back in October for my 6 month pap it came back normal and negative ofr High Risk HPV. I went in April 2008 for another 6 mont pap and it came back abnormal showing ASCUS and poitive for high risk HPV. My docotor said she wanted to wait four months and do a repeat pap smear. I was not comfortable with that so I went and got a second opinion and the doctor examined me and starting telling me that my cervix would have to be stitched if I ever wanted to have another child because it did not heal correctly and I took a copy of my pap results and he stated that my results also showed yeast infection which my orignal doctor did not tell me. He stated that it would be very difficult to do a colposcopy because of the way my cervix had healed and that he wanted to do a colposcopy in June. I was really upset and I called my original doctor and explained to her what he had said and she stated that there was no way you could tell if your cervix needed to be stitched before becoming pregnat, I would like to know if that is true or not. I am going back in August to see my original doctor to do a repeat pap and if I am not comfortable with the results or the way she handles the results, I am going to swtich to my sisters doctor that is the same office. My biggest concern is the waiting game, should I tell her to go ahead and do a colposcopy or do a repeat pap and see or should I not wait at all and find another doctor. I am not real clear as to what ASCUS really means, she was not very imformative all she said was that there was some slight changes but nothing major and that is why she wants to wait four months. Any advice would be great. I want to have another child but I am so terrified that if they have to keep doing these procedures I will not be able too.

Welcome to the boards. I'm sure someone will come along and be able to answer all of your questions. Right now, I only have the following to offer.

The only advice I have right now is to make a list of questions to take with you to the doctor's office. I also suggest you get a 3rd doctor; preferably someone who will answer your questions.

Hi..sorry for the delayed response. I have a history of ASCUS results but negative for HPV. ASCUS means Atypical Squamous Cells of Undetermined Significance. This is the mildest grade of abnormality. It's borderline normal but not quite. If you are HPV negative twice, Dr's will generally suggest you return to annual exams (although I probably wouldn't accept that...maybe 6 mos but not a year). Anyway, if you are ASCUS/HPV positive, I believe they repeat the pap in 3 mos and follow you closely. If at 3 mos (or 4 in your case) you are still abnormal with HPV positive, they will likely proceed to LEEP again.

I do not know the risks with regard to pregnancy and stitching etc. I've heard of Dr's encouraging patients to hurry and get pregnant followed by a more testing and/or invasive procedure (hysto). Although, like you, it was difficult to wait 3 mos for my repeat pap, I can't imagine waiting 9 mos for a pregnancy to pass knowing something needs attention. For me, that would make for a stressful pregnancy. In any case, I am sure for gyne's this is manageable so like Pickle suggested I would be frank with the Dr and ask him/her what your options are and the risks associated with each option.

Thank you for your advice, I will be taking in a list of questions when I go back in August. I am not to educated on all of this when it comes to abnormal paps/High risk HPV. I have not had any problems until April of last year then in October my pap and hpv came back normal and negative and thensix months later it is positive and abnormal. What would be the main questions that I should ask.

Since you are HPV positive, I would ask him to repeat the pap of course but I would try to push for a colposcopy with ECC (endocervical curretage). A pap is a screening tool while a colposcopy is diagnostic. Your cervix is similiar to a doughnut except the hole in the middle is tight. The colposcopy looks closely at the surface of the doughnut. The ECC is a scraping from inside the doughnut hole. So even if the surface looks good and does not require biopsy, you should ask them to do the ECC no matter what just to be more thorough.

I would also ask what his follow up plan will be (3 mos, 6 mos etc.). I would not accept more than 6 probably. I would ask the effect of LEEP on carrying a pregancy to term. I would ask if it would make sense to try to get pregnant now while things seem under control if that is the case. I'd also want to know what effect pregnancy can have on my condition...could it cause it to progress faster due to hormones involved? I would ask what the treatment options are for each level dysplasia.

Okay I am freaking out....I'm 22 years old and am schedualed for my second leep procedure on July 17th I just found out I am pregnant and have had a miscarriage and an abortion after serious medical problems ....will I be okay to carry a baby to full term? Should I still have the leep? I'm so scared....HELP!!!
