Please help me figure this out

Hi everyone, I am new on here and this is the first time I've posted anything. I am a 49 year old female. I've been having stomach and bowel issues most of my life, I was constipated for most it and then began the cycle of diarrhea and than constipated, always needing to know where the bathroom is at any place I go. I pretty much diagnosed myself with IBS, no big deal, deal with it! I had gastric bypass surgery about 10 years ago and it went very well, however I continued to have issues (which I expected). However in the last several months things have continually gotten worse with the onset of different symptoms. Beginning with overall weakness, tired, have always had insomnia, joint and muscle pain, my hips and lower back are the worst right now. Then a couple of months ago I began having alot issues with my bowels and stomach. It started with pain and cramps in my stomach, accompanied by a lot of diarrhea (sometimes straight liquid) alot of gas and loud rumbling in my stomach, and when I do go and it's not diarrhea it is small pieces between the size of my pinky and my index finger. The last couple of weeks I have had more severe stomach cramps and sometimes a sharp pain on my left side. I also feel a heaviness in my rectum and I feel like I've never completely emptied myself. Upon researching these symptoms I became concerned, however other than a few small traces of blood on my tissie there hasn't appeared to be alot of blood loss, I actually attributed what I did have to hemmroids. Anyway I made an appt to see a doctor through my new insurance, Kaiser. I guess it was just meet and greet type thing because I wasn't even examined or really even given the chance to voice all of my concerns. She did do blood work and I saw the results this morning. Can you all tell me based on this information if I am anemic? And if so could this be due to blood loss that I'm just not noticing? My stats are:

Ferritin -8
Iron level - 24
TIBC - 419
B12 - 193

I am so frustrated right now, I'm tired of this diarrhea, it is nothing for me to go 6-7 times a day. I'm going to make my appt for my physical but I would welcome anybodies thoughts or suggestions, I don't know why I can't ever find a doctor that truly listens to me. I know my body and I know something isn't right, how do I convince them of this. They make me feel like I'm a hypocondriac.

Also, a note, I understand as a gastric bypass patient that my vitamins, etc don't get properly absorbed, that's why I take vitamins, but in the last 10 years since my surgery my iron levels have always been normal....thanks

Dear jenzfreedom~

My understanding of Kasier is they require your primary care to act as the gatekeeper. Based on your symptoms, you may want to request a referral to a gastroenterologist. I am sure you feel despair, and when you do get 90 seconds with your provider you feel compelled to rapid-fire a litany of issues in the hopes s/he will take you seriously.

If I might, I encourage you to make both a mental and actual list of the top three things you most want to address with your provider. Try to be as emotionless as possible....this is business for yourself. It is after ALL about you and seek the answers you desire. You have a problem, and you want a resolution. If you do not believe your current provider is meeting your needs, try and find another. I know some managed care organizations allow you to see providers outside the system for a price...sometimes an annual fee or a premium per visit. If you are able to afford an increase out-of-pocket cost, you may consider that. Don't allow the system to mandate your answers or future. Do you have specific diagnoses/concerns in mind? If so, ask you provider what steps you could take to rule it out.

You are correct in that gastric bypass patients do not generally absorb nutrients as typical patients and require supplementation. Your labs appear abnormal, but without the reference range one cannot accurately assess severity of abnormality.

Thank u Hannah, that is what my husband is telling me, if I don't feel confident with my doctor find another one, which I will do

Hi Jennifer,

I totally sympathize with you. It's so hard to talk about yourself to a doctor and try to get them to "hear" you. I couldn't tell if this was your first visit with this doctor or what but I will tell you, just from my own experience, you have a "system" that sounds very much like me. Your family history is very important and you didn't address that so that would be something this doctor should know. I have a family history of colon cancer and most of my adult life I went back and forth with constipation or diarrhea and just figured it was the way I was. At your age, I highly recommend you see a gastroenterologist asap. Educate yourself, find out what you need to ask these doctors and make sure they take care of you. Unfortunately, in this day and age, that's what it takes to make sure you get the best care. If you are in an HMO, get a referral to a gastro doc. Best case scenario for you is to find what exactly is going on with you and ease your mind about "what if's". I don't want to scare you and say anything else until you get a chance to get some good answers ok?

I am new to this site as well. I saw your question and just wanted to try and help. I am no doctor of course but I know allot about this. Hugs!!!

Thank you so much for your reply. I have been to the doctors twice, it was with an internist who believes the problem lies somewhere in my spine?? On top of the bowel problemts I also been experiencing pain in my hips, lower back and buttucks, also down my left leg when I stand up, it just all aches when Im sitting. My blood work came back fine except that my B-12 levels were low, my iron was low and my Vit. D was low. They had me have an xray yesterday. I really like my doctor, but she seems to use medication for everything, that is bothering me. I had been put on Wellbutrin first. When explaining my pain and frustration with her I began to cry and from that point on she is giving me a different prescription any time I mention a concern. I am now on wellbutrin with serzone, neurotin as a nerve blocker, flexeril for the muscle aches, tramadol for the pain and clonozipan for the stress and anxiety. I feel complety doped up but still in pain, so I'm not sure what to do? I go to the spine specialist this Friday so we will see what he says. Jennifer,

Too many meds, IMHO, especially if they're not helping. Sounds like you could be having some sciatica issues and/or piriformis syndrome. I've had some of the same pains in my buttocks, legs, hips. Had xrays, MRI, everything normal. I think mine is from sitting too much, especially at work . The stomach issues could be a result of your gastric bypass, but it actually sounds like IBS. Hopefully the spine doctor will have some answers for you. Good luck.

Thanks for you input, alot of what you are saying is making sense. Everyone around me are noticing the change in my behavior and I'm not sure what to do, I guess the best thing to do is call my doctor, one of the meds is making me have hallucinations and I'm not sure which one? I'll let you know how things go with the spine specialist.

Jennifer,

Please research drug interactions of the meds you're on. Several of them lower the seizure threshold and have major interaction. Some of the others have moderate interaction. I don't understand why doctors prescribe so many meds without doing more research. Be your own advocate....take care.

Jennifer, boy you are on allot of meds and unfortunately some doctors don't always realize what you need. Usually those drugs are prescribed by a psychiatrist not a regular MD. Unless you have depression issues in your family and this is new for you, i.e. crying and stress, I would see a specialist before taking all those drugs. Of course, you're in pain and that is VERY depressing and stressful. You may even have hormone or other imbalances causing problems. Just knowing there are good and bad doctors, it is well worth your time to get recommendations and find a doctor who will check everything before just giving you a prescription. I hope the spine doctor helps you out. All I know is, when you have the type of problems you are having there should be testing done until it is figured out. Blood work is fine but the doctor decides what the test is looking for and something important may be missed. I sure hope you feel better soon. I know it's frustrating to go to a doctor and not get any answers and feel as bad as when you walked in.

Your symptoms sound very similar to what I went through . All of your stomach issues. It wasnt until i read in one of your responses to another comment that it really really sounded like what I had ! The pain in hips , buttocks , down the leg , combined with the abdominal problems was me to a T. I went to every kind of doctor to no avail . That is until I went to my gynecologist . Come to find out , I had an ovarian cyst. The cyst at times was pushing on my bowels, actually pinching it , which would be the times I was constipated or always felt the need to go or the feeling of never finishing. Stomach pains with Diarrhea would come and go but yes daily. The lower back pain , what I call hip pain/burning as well as my leg leaved me to believe it was spine back related also. If you have not been , please go and insist on an ultrasound to see if this could be causing your problems . Mine was not very big either though it did grow before finally having it removed . Since my surgery , all of those symptoms and I mean ALL have gone and not returned . Every one of your symptoms fits my diagnosis perfectly . Hope this helps and maybe gives you some other direction to go.

Thank you momoftrio for your reply, it actually makes sense. I am due for my yearly checkup anyways, so I will make an appt, but how do I convince the doctor that I need a untrasound? They won't just do one for no reason will they? I went last year and had an exam, pap, mamo and everything was fine, but this has all come up since then, once I go I will let you know, again thanks for the info...Jen

I would think if you just mentioned the fact that a friend ha the same exact symptoms and an ultrasound revealed a cyst . When my cyst was found it was only 3.5 cm. but from what my doctor said . The size really didn't matter considering I was so uncomfortable from it . The position it was in and where is was pressing was obviously an issue . If you ask for one to rule out any gynecological problems I'm sure they will do it . Just be adamant about it .
