| Gleason 9 with mets - Orchiectomy + Triple Blockade

My father was diagonised with PC Gleason 8,9, PSA 164 and showing a spot on D12. The trigger was a one off PSA. We went for further tests 8 weeks back,did an MRI which showed engagement on the Lymph node biopsy 6 weeks and intial biopsy showed a Gleason 5,6 and well differentiated cells and negative technitium bone scan. My dad chose the orchiectomy option and had the surgery with radiation suggested after 4 weeks. However the PSA was still 15 in 4 weeks so we did a recalibration. We found a spot on D12 (F18 Bonsescan) and a revised gleason of 8,9.
My dad has been advised a wait and watch approach and say that the orchiectomy is comprehensive. I am not able to get any experiences on how to enhance the orchiectomies effect. Should we add a hormonal therapy to this?. Is it worthwhile radiating the spot.
My dad is most positive and wants to focus on his life but we are very worried that the wait and watch is a passive approach very unlike how many here are handling their disease. Would really appreciate your experiences[/COLOR][/COLOR]
