When prostate cancer metastasizes, it often travels to the bones, especially those in the pelvic area (pelvis, spine, femur, etc.). It is unusual for a metastasis to only occur as long as three years after RP. Has his PSA been rising? What is his PSA now? Was he diagnosed with a bone met? Why is this a concern now?
