I've found that GPs are not very good at giving DREs. Some of them are as uncomfortable doing it as we are receiving it and don't take the time to feel around properly. I had a GP and a proctologist tell me they felt nothing and two urologists told me they felt a lump (The urologists were right.)
Your rapidly rising PSA may be prostatitis, it may be the result of urinary tract obstruction, or it may be prostate cancer (BPH typically doesn't make PSA rise that high that quickly). Your urologist might give you 3 weeks of Cipro to see if your PSA drops, indicating prostatitis. He might also check for urinary retention. If he rules out those causes, he might want to schedule a biopsy.
