I have a slight underbite that an ortho thinks can be corrected with braces. I also have severe TMJ pain. Depending on the severity of the underbite braces may be all that's needed, or surgery will be recommended, but I doubt any doctor would recommend extensive treatment before permanent teeth are in, unless there is serious trouble with the jaw joint itself early on. The growth of the jaws during adolescence will also affect how your child does-I was fine as a child, but my lower jaw outgrew my upper during puberty.
