I have a 10 year-old boy that had testicular pain last month. He had torsion of the appendix of the right testicle, which fortunately is different from testicular torsion, which requires emergency surgery (due to risk of losing testes function). The doctor was an amazing female doctor, i stayed in the room and my son was in so much pain he couldn't care less if she was a male or female. Talk to your son or ask him to read these posts, because he can't risk his health out of pure embarrassment. I don't want to scare you, but I had a friend who was in his teen years and he was suffering from testes swelling. He felt embarrassed to tell/show his parents. Unfortunately when another family member noticed the difference in size it was too late, he had cancer. I'm NOT saying that this is your boy's case at all, but I never forgot this true story because eventually he died (cancer spread to other parts). Just tell him healthcare professionals should be seen as sexless , they're only trying to help. In my son's case, tons of Advil, rest and 2 medical appts were enough to heal him (he's alive and kicking). Good luck! Any questions, we're here!
