I feel your pain, I wasn't able to smile for years. And people always pointed out that I never smile with my teeth. I finally got some of them fixed and I'm a smiling freak, haha.
The stuff that looks like sand under your gums sounds like an infection to me. (Of course I'm not a doc, just my opinion) The part that looks like sand, is it sensitive?
I would go to the dentist asap to get it taken care of. If you need to lose the tooth, there are many options to replace it. And there are temporary bondings also I believe. So don't stress, there are solutions... you just have to get to the dentist before the infection gets worse or you start feeling pain. The last thing you want is a toothache, I've lived with them most of my life and they're awful!
