It is a combination of bits of food, mucus, sinus drainage, etc., that collect over time. For me, it also occurs pretty rarely but once, I kept on tasting and smelling a horrible odor and taste in my mouth that was a lot like tooth decay. I flossed, brushed, tongue scrapped, mouthwashed, but it didn't go away. Then, I looked down my throat and saw a large amount of that white nasty stuff protuding from a large "crypt". I pressed against it with a Q-tip and --Eww!--gobs of cottage cheese-like stuff that tasted and smelled like a stinking corpse gunked out. Once it did, my breath was fresh again. A friend who knows a dental hygienist told me to gargle with diluted hydrogen peroxide every two to three days. This will dissolve whatever is fermenting in those little holes. DO NOT SWALLOW IT!! Note: this won't work on those tonsil stones, which are a different animal altogether. For this, you need an ENT to remove them. Sorry.
