My experience with pain is that sometimes it takes a high dose intitally (of ibuprofen) to alleviate the pain. If you have ever had it prescribed you are aware of how high the doses can be than OTC. I take a higher dose then take lesser doses every couple hours. Once I feel relief I space out the doses to every four hours. Mind you, this is in one day. I would be concerned if I'm not feeling some relief after this because it could mean something else is going on. I would expect to feel considerably better after a few days
Also, I take a product like Pepcid 30 minutes before the higher dose and then a couple more times (3 doses in one day.) This was recommended by my doctor because my stomach gets "torn up" otherwise (from the ibuprofen).
