What holds a distal extension partial in place?

I have to get top and bottom partial plates b/c of numerous extractions. On the top one I will have teeth to clasp the partial to but on the bottom I'm only going to have teeth to clasp too one 1 side therefore the dentist said I would need a distal extension partial. I'm only 25 and know nothing about partial plates... what type of partial is this and what is going to hold it in place? Is it going to slip out of my mouth or is it going to be noticeable? Any help would be greatly appreciated... I'm already required oral sedation b/c I'm so freaked out by the extractions and now adding this on top of it i'm really worried! Thanks so much!

Also is there going to be metal visible in my mouth? Especially with the distal extension.. something's got to hold it in and if there's no molar's on 1 side of my mouth I'm scared there's going to be metal showing front the front teeth or something!
