Are you possibly facing a divorce, or wondering about all this because it is scary to take psych meds and not know what they may mean in the long run? You are allowed by law to sit and read your own medical records. I'm not sure if you need any special forms but that probably varies by doctor's office or insurance provider. Ask your doctor.
If there is a medical reason for you to take meds and you are taking them as your doctor prescribes them, then there is no reason for guardianship to be removed, as far as I know.
Maybe if there were lots of hospital stays for mental illness and you had no immediate family (parents, sisters, brothers) who could care for the kids then there may be reason for at least joint custody. I'm not sure about losing custody of the children completely.
How old are your children? What do you think your doctor has you taking meds for? And do they help you at all? (you don't have to answer anything you don't want, of course). Take care, and post again. It sounds like you're in a difficult spot right now.
