Diabetes-medication-constipation?

My brother is diabetic, I guess it's pretty severe as he has to take insulin several times a day. He also takes other meds for various problems like neuropathy, etc.
He has a really bad time with bowel movements. He's tried all the usual fixes, diet, laxatives, prunes, stool softeners. Nothing is effective and it's really an awful problem for him.
I heard about laxative tea that worked pretty well, but would like to hear some results that you all may have had.

Couple of ideas :

Hydration makes a huge difference. We all think we're getting enough water, but there is no such thing as too much. Try over-hydrating for two weeks.

The high fiber foods make a big difference for me. I started eated Lentils for their high fiber / low GI value. The result was much more regularity.

I went from two bowel movements a week, to daily just increasing my hydration and 35 or more grams of fiber a day.

Softeners did not help the frequency but they did make it less painful when it did come out. I don't use them anymore since the frequency is resolved.

One other suggestion is to try a tablespoon of ground flaxseed every day. You can buy ground flaxseed at a health food store. Sprinkle it over yogurt or incorporate it into a smoothie. It works very well and its high fiber content also slows down the absorption of carbs/sugar.
