You clearly have some pretty significant insulin resistance. What's happening is that your body is needing to release a large amount of insulin to cover the carbs you are eating. Because you need more insulin, it takes longer for the insulin to kick in, allowing your blood sugar to spike high after a meal, usually within the first hour or two, then producing a rapid drop in blood sugar. That's most likely what caused the 30 on your glucose tolerance test too. Speak to your doctor about the best course of action regarding dietary changes.
